Led by Kano, the Black Dragon clan was a ruthless crime organization feared around the world by both law enforcement, as well as other crime syndicates. It was formed by ex-members of the Red Dragon clan who disagreed with its strict code enforcement.

The Special Forces, led by Sonya Blade, was created by the United States government to help disrupt the Black Dragon's criminal activities. Members of the Black Dragon were also marked for death by the Red Dragon.

During the events of the first Mortal Kombat tournament, Kano was able to lure Sonya Blade and her team to Shang Tsung's island where she was forced to compete. After Shang Tsung was defeated, he captured Sonya and Kano and brought them to Outworld in an attempt to lure Earthrealm's warriors there. Jax was able to rescue both Kano and Sonya. However, Kano escaped and managed to convince Shao Kahn to spare his life, eventually, becoming a general in Shao Kahn's army.

Most thought Kano to be dead, and the Black Dragon began to fall apart. Many of it's members disappeared, were arrested, and/or killed. However, Mavado, leader of the Red Dragon clan, was told of Kano's whereabouts by the Deadly Alliance and given an opportunity to capture him. Successful, Mavado brought Kano back to the Red Dragon's mountain headquarters where he was experimented on in their attempts to create human-dragon hybrids.

With Havik's advice, Kabal saw an opportunity and decided to take control of the Black Dragon clan. Kabal had once been a member of the Black Dragon, but left criminal life after almost being killed by Shao Kahn's extermination squads, during the events of Mortal Kombat 3. He recruited skillful, young warriors, such as Kobra and Kira, to help rebuild the clan's numbers and restore the clan to power.
